Anova Vacancies – Outreach Team Leader: Professional Nurse

Job Description:

Anova is a leading NGO dedicated to empowering people and transforming lives through quality healthcare solutions. We are seeking a motivated and dynamic Outreach Team Leader: Professional Nurse to oversee Home-Based Carers, CDU, CDL, Rehab Adherence, and CDU Staff.

Key Duties and Responsibilities:

• Develop weekly work plans for Community Health Workers (CHWs) and personal tasks (home visits, household assessments, community outreach events, etc.).
• Manage Community Health Workers in each node.
• Collaborate with DoH facility management and community leaders to support the Community-Oriented Primary Care approach, including attending local community meetings.
• Participate actively in multi-disciplinary meetings organized by DoH facility management.
• Engage in wellness and prevention initiatives, such as administering vaccines, screening for non-communicable diseases, and family planning.
• Plan and conduct mentorship and coaching sessions for CHWs on the Integrated Health and Wellness Service package.
• Maintain and update patient files and statistics regularly.
• Attend training sessions organized by DoH and/or Anova.
• Travel to Anova wellness outreach venues and/or DoH facilities in the Cape Metropole District as needed.
• Review and correct CHWs’ registers and reports.
• Provide accurate verbal and written reports on statistics and narratives.
• Ensure patient confidentiality and comply with POPIA regulations for storing patient files.
• Manage CHW timesheets, leave, and submit to Anova’s HR department.
• Handle office expenses, advance claims, and mileage claims, ensuring timely submission to Anova’s finance department.
• Prioritize, organize, and manage workload to maintain and enhance quality.
• Perform any additional tasks as agreed with the line manager.

Minimum Qualifications, Experience, and Skills Required:

• Nursing qualification (Professional Nurse).
• SANC registration.
• Valid driver’s license and own transport.
• Experience in community work is advantageous.
• Positions available in Khayelitsha, Parow, Westlake, and Strandfontein. Please consider location when applying.
